Release and Wavier of Liability for Open Doors Improv Camp Presented by Shut The Front Door Social Group
THIS IS A VOLUNTARY RELEASE OF ALL CLAIMS BY YOU. PLEASE READ CAREFULLY BEFORE SIGNING.
I, the undersigned, (hereinafter “Camper”) declare that I am eighteen (18) years of age or older, and execute this waiver with full understanding of its meaning and of its legal consequences. I hereby expressly release Shut The Front Door Social Group, its directors, owners, officers, agents, employees, contractors, volunteer, team members, partners, sponsors, successors and affiliates of any claims arising from the use of facilities and equipment provided by Stevenson Children’s Camp and Shut The Front Door Social Group.

I, the undersigned Camper, desires to attend Shut The Front Door Social Group’s Open Doors Improv Camp and may engage in the activities related to being a camper, and understand the nature of such activities. These activities include transportation to and from Stevenson Children’s Camp, regardless of whether such transportation is provided by Camper, Shut The Front Door Social Group, or a third party.
In consideration of my participation in this Camp, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
1. Waiver and Release.
I release and forever discharge and hold harmless Shut The Front Door Social Group and their respective, directors, owners, officers, agents, employees, contractors, volunteer, team members, partners, sponsors, successors and affiliates (hereinafter “Releasees”) from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my attendance at Stevenson Children’s Camp or participation with Shut The Front Door Social Group’s Open Doors Improv Camp.
2. Medical Treatment and Insurance
I hereby release and forever discharge the Releasees from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered to me in connection with an emergency during my attendance at Open Doors Improv Camp.
I AM SOLELY RESPONSIBLE to select and purchase adequate medical/health insurance. The Releasees will provide no medical/health insurance. In the event of a medical/health problem, the Releasees accept no responsibility for any costs associated with a medical/health problem nor will they pay for any medical/health expenses that may be incurred by the participant.
I freely accept and assume all responsibility to provide myself with medical/health insurance coverage.

3. Assumption of Risk and Indemnification for Alcohol Consumption
I understand that at no point of the Camp will the Releasee be providing or serving alcohol to any Camp participants. Provided that I am of legal age and bring alcohol to the Camp, I understand that inherent risks arise from the consumption of alcohol, including but not limited to: bodily injury to others or myself, illness, property damage and death. I understand that consumption of alcohol is completely voluntary and I must be 19 years of age or older to consume alcohol legally in Ontario. I hereby expressly and specifically assume the risk of injury or harm in these activities and release Shut The Front Door Social Group from all liability for injury, illness, death, or property damage resulting from my activities and my time at Open Doors Improv Camp. If I, or anyone on my behalf makes a claim against Shut The Front Door Social Group related to my activities and attendance to Open Doors Improv Camp, I agree to indemnify, save and hold harmless Shut The Front Door Improv Social Group from any claim, including without limitation, loss, liability, damage or cost which may occur as a result of any such claim.

4. Assumption of the Risk and Indemnification for Camp Activities
I understand that attendance at Open Doors Improv Camp may include activities that may be hazardous to me, including, but not limited to: hiking, swimming, team sports, rock climbing, dancing, cooking on an open fire, low ropes courses, team building communication activities, improv workshops, and other activities associated with the camp experience. These inherent risks include, but are not limited to: encountering natural dangers such as irregular or uneven ground, unseen and unmarked objects, drowning or serious injury in and around water, including injuries due to subsurface conditions, exposure to cold water, water impurities and the like. Inherent risks also include acts or omissions of other campers, sunburn, rain or hail storms, tornadoes and the like, contact with plants, animals or insects, the risk of Camper engaging in unauthorized activities, Camper’s physical conditions or Camper’s own acts and omissions, conditions occurring on trails, roads, waterways or terrain, the administration and availability of first aid and emergency treatment and consumption of food or drink by Camper. I understand that participation in these activities is completely voluntary. I hereby expressly assume the risk of injury or harm in these activities and release the Releasees from all liability for injury, illness, death, or property damage resulting from my activities and my time at Open Doors Improv Camp. If I, or anyone on my behalf makes a claim against Shut The Front Door Social Group related to my activities and attendance at Open Doors Improv Camp, I agree to indemnify, save and hold harmless Shut The Front Door Improv Social Group from any claim, including without limitation, loss, liability, damage or cost which may occur as a result of any such claim.

5. Photographic Release.
I grant to the Releasees the right to take photographs and video recordings of me and my property in connection with the above-identified subject. I hereby grant a royalty free license to Releasees for the use of my likeness captured within any image, audio or video recording. I agree that Releasees may use such photographs and video recordings of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

6. Severability
I agree that in the event that any court of competent jurisdiction would deem any clause or provision in this Waiver invalid, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release, which shall continue to be enforceable.
ACKNOWLEDGEMENT
I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators and representatives;
I understand that participation in the Camp and related activities could involve risk of physical injury, illness, death or property loss, and despite safety precautions, the Releasees cannot guarantee safety thereof, as all risks cannot be prevented. Releasees do not provide health and accident insurance for Campers, and I understand that any medical expenses, property loss, or other personal expenditures that result during or from this Camp, are to be borne by Camper. Camper hereby consents, give authorization to, and release from liability; trip leaders to secure any emergency medical treatment in event I am unable to, and I agree to be responsible for the costs thereof.
I HAVE READ THE ABOVE WAVIER AND RELEASE, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT VOLUNTARILY.
